Black Cohosh side effects:
Mild black cohosh side effects of headache, nausea, dizziness, etc have been mentioned but thus far, and rare incidents of potential liver problems.

Caution: In rare cases black cohosh supplement use has been reported to affect the liver. Discontinue use and consult a healthcare practitioner if you have a liver disorder or develop symptoms of liver trouble, such as abdominal pain, dark urine, or jaundice. If you plan to take black cohosh for prolonged periods, take a week off each month.

Black Cohosh dosage and availability
Black cohosh supplement is sold either by itself, or combined with other herbs and nutrients. The dosage of black cohosh extract used in the majority of clinical studies has  been based on the level of a key marker, 27-deoxyactein. The recommended black cohosh dosage is one tablet of 20 mg taken twice daily, or one 40 mg capsule daily.
