Q. Where does carnitine come from when it is in a supplement form?
A. Although carnitine is a naturally occurring molecule in all living cells, in general commercially available carnitine sources such as acetyl L carnitine and carnitine tartrate are synthetically made by the chemical industry in the US.
Q. I am interested in taking a carnitine supplement but not sure whether acetyl l-carnitine would be preferable.
A. When ingested as a pill, carnitine is not able to cross the blood-brain barrier as well as its activated form acetyl l-carnitine which has a more noticeable effect on the mind than carnitine. The acetyl form usually enhances mental clarity and focus, along with slight mood elevation. The effects of carnitine are more physical than mental, in the sense that carnitine enhances physical energy whereas acetyl l carnitine provides more mental energy.
Q. Is carnitine an antioxidant? Which is a better antioxidant to take?
A. Both acetyl l-carnitine and carnitine are excellent antioxidants, although the former is more potent.

Frailty is a geriatric syndrome characterized by muscle weakness, sarcopenia, and fatigue, and is associated with several adverse health outcomes, including disability. Carnitine is important for energy production in skeletal muscles and there seems to be a negative correlation between advancing age and muscle carnitine levels. Carnitine deficiency may therefore result in geriatric frailty. Age-associated carnitine deficiency from a variety of etiologies may potentially explain the relationship between carnitine-associated mitochondrial dysfunction and geriatric frailty. Development of therapeutic agents capable of prevention or reversal of carnitine deficiency in older adults may minimize the occurrence of frailty in geriatric populations.

Reduced circulating and tissue carnitine levels, possibly leading to impaired mitochondrial function, have been postulated to be involved in the pathogenesis of insulin resistance. However, whether L-carnitine administration may improve insulin sensitivity in patients with impaired fasting glucose (IFG) or type 2 diabetes mellitus (DM-2) is still controversial. The aim of the study was to explore the role of L-carnitine supplementation in influencing insulin sensitivity. Patients were randomly assigned to receive a hypocaloric diet for 10 days (group C) or the same dietetic regimen in addition to oral L-carnitine (2 g twice daily) supplementation (group LC). Oral glucose tolerance test (OGTT), fasting plasma insulin levels, and homeostasis model assessment of insulin resistance (HOMA-IR) were assessed at the beginning and end of the study. OGTT at 2 hours improved in both groups. Only in the L-carnitine supplemented group did plasma insulin levels and HOMA-IR significantly decrease when compared to baseline values. Oral L-carnitine administration, when associated with a hypocaloric feeding regimen, improves insulin resistance.
